To err is human

There is a fallacy that people, particularly health professionals, never make a mistake.

This is, of course, absurd. To be human is to be fallible.

To quote my Dad:

"Only one person has ever walked this earth and not made a mistake and I'm pretty sure it wasn't you"

We are therefore left in the position of either being someone who pretends that they never make a mistake or someone who understands that they make mistakes identifies then confront them learn from them.
